If you’re planning to renew your New York State driver license, apply for a new license, or obtain a non-driver photo ID card, you have the opportunity to register as an organ donor.
Becoming an organ donor at the Department of Motor Vehicles (DMV) is simple.
When you fill out your driver license or non-driver ID card application (form MV-44), there will be an area in which you can sign to enroll in the New York State Department of Health’s Donate Life Registry:
Just sign and date this section.
Your signature indicates you are legally consenting to be an organ, tissue and eye donor. It authorizes the DMV to give your information to the New York State Department of Health for enrollment in the Donate Life Registry.
You will receive a letter of confirmation from the Department of Health, which will also provide you with an opportunity to limit your donation.
